How do I schedule an appointment?

How should I prepare for my appointment?

In order to provide you with the best possible service, we encourage you to sign up for the Patient Portal to complete your pre-visit forms. The portal allows you to share your relevant medical history and background information (such as current medications and lab results) with us prior to your appointment. By taking care of the required paperwork ahead of time, we will be able to maximize our time together and focus on what matter most: your care.  

On the day of your appointment:
1. We recommend that you plan to arrive at least 15 minutes before your appointment time. 
2. Please bring your insurance cards, valid ID, and credit card. 
3. We ask that you bring previous laboratory results or relevant health care records to your appointment if you would like the physician to review them.  

Note: If forms are not completed prior to appointment, the average time to register and fill out forms is approximately 15-20 minutes, which will be taken away from your appointment time.


How do I sign up for the Patient Portal?

When you call 773.377.8000 to make an appointment, you will be given the opportunity to provide your email address in order to complete registration for the Patient Portal. Once you have set up your access, you can input your relevant medical information into the system. If you did not sign up for the Patient Portal when you made your initial appointment, you may still do so before your office visit by calling 773.377.8000 and providing your email address so you can register for access to the portal.


How long is my appointment?

New patient visit is 45 minutes
Follow-up visit is 30 minutes

Acute Care visit is 15-30 minutes (depending on complexity)

Initial I-Squared Life Member visit is 120 minutes
I-Squared Life Member Follow-up visit is 60-90 minutes


What is your cancellation or late show policy?

If you arrive 15 minutes or more after your scheduled appointment time for a regular followup or you arrive 10 minutes or more after your scheduled acute visit, your appointment may need to be rescheduled or you may have a longer wait. In the event that you do no show or do not cancel less than 24 business hours prior to your appointment, you will be charged $75.00. 

 

Do you take insurance?

Currently, we accept most major PPO plans and Medicare. We do not accept any HMO or Medicaid at this time. We also provide fee for service care if we do not accept your insurance. Please contact the office for the fee for service charges. Because every plan is different, please be aware of your deductible. If we are not contracted with your insurance and you have out of network benefits, as a courtesy we will submit your superbill to insurance for direct reimbursement to you. But you will be required to pay for your visit at the time of service. We will give you an estimate of the charge at the time of your visit. We will be re-evaluating insurance on an ongoing basis, please update us if you have any change in your insurance coverage. 


What is my insurance deductible?

The deductible is a specific amount you must accumulate and pay each year before benefits are provided by your carrier. Please be aware of your yearly individual or family deductible which may explain why you are receiving a bill from office for your visit to cover the portion of services not covered by your insurance.

Why do I need to have a credit card on file?

We are requiring a credit card on file to streamline patient billing, collect cancellations fees, to pay for services like massage, acupuncture, nutrition, and supplement sales. If there is an outstanding balance owed, I-Squared Medical Group/ I-Squared Life will notify you via phone and or email. If by the final billing notice we do not receive a response from you or your payment in full, at that time, any balance owed will be charged to your credit card. A copy of the charge will be sent by email or mailed to you.  


How do I pay a balance on my account?

To pay balance on your account call the office at 773.377.8000. We take credit / debit cards, check or cash for balances due.

You can also pay via Patient Portal.

If payment is not made within 30 days after the second notice of your invoice, your credit card on file will be charged the past due balance.  


What is the best way to communicate with the practice?

The best way to communicate with us is via Klara, our messaging platform. You will find a link to Klara on the home page of our website. You can also initiate messaging by calling the clinic and pressing “0”. Another way to communicate with us is via our Patient Portal. When you call to schedule your first appointment, you will be given the opportunity to provide your email address in order to register for access to the Patient Portal. If you did not sign up for the Patient Portal when you made your first appointment, you may still do so by calling 773.377.8000 and providing your email address to register.

The portal is to be used for non-emergency communication only. If you are having an emergency, please call 911 or call the clinic directly. 

If you are having trouble accessing or registering for the portal, please call the office and we will be more than happy to help. 

 

What are the hours of operation of the clinic?

Monday – Friday: 8am – 5pm
Saturday & Sunday: Closed

We are closed on major federal holidays, including Martin Luther King Jr. Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Thanksgiving Day, Christmas Day and New Year’s Day.

 

What if I call after clinic hours?

We will answer post-clinic hour calls, however please be aware that you will be charged a $50.00 fee.

Note: For I-Squared Life members, the fee will be waived. If you are calling with regard to an emergency, we encourage you to visit your local Emergency Room as soon as possible.


What happens if I have an emergency?

In the event that you have an emergency, please call the office. However, do not hesitate to call 911 or go to the nearest Emergency Room to expedite your care.


Is there parking? 

The parking garage is located on the west side of the building off Sheffield Avenue. We offer 1-hour validations for the first hour, for 2-hours $15, 3-hours ­$16, 4-hours $21, and more than 4-hours $26. There is metered parking on the street as well.  

 

Can I order supplements over the phone or online?

Yes, please contact the office at 773.377.8000 ext. 3 for assistance in placing your order. You may also order supplements online by visiting our apothecary here. Be sure to enter Access Code: I2Life when placing your order. You will receive $5 off of your first order.

 

What is your return policy on supplements?

If you have your original receipt and supplements were bought within 1 week, please bring the product back to the office and it can be returned for full credit. If the product was purchased through our on-line apothecary, please contact them for assistance with your return. If the product has been opened, we unfortunately cannot accept a return, unless you had an adverse reaction to the supplement. In that case, if you bring the product back to the office within 2 weeks of purchase, you will receive office credit for future supplement purchases. Returns will not be accepted without the original purchase receipt.   

 

Is there a fee for getting medical records transferred, medical forms to be completed, refill authorizations, or letters for work?

There is a $15.00 fee for medical forms or letters. There is only a fee on medication renewals for controlled substances like pain medications or stimulants or when antibiotics are needed. There is no fee for I-Squared Life members.

What specialty services are offered at I-Squared Medical Group?

We offer Massage Therapy + Acupuncture, Functional and Board Certified Nutritionist consultation, Massage Therapy + Reiki / Energy Work (may be reimbursable to you by submitting your superbill to insurance—please contact your insurance carrier for instructions on where to send the superbill / receipt for services), IV therapies, and Infrared Sauna Treatments. To streamline your experience, we request your payment prior to your visit or when you check in for your service. If there are any additional services done at the time of your treatment, you can pay for that after your service is completed.

  

How much are IV treatments?

We have standard IV treatments and customized IV treatments. Please call the office at 773.377.8000 for more information.

 

What if I need to be admitted to the hospital?

Due to the changing nature of medicine, you will be admitted to the hospital service where you are being evaluated. Please have the admitting team contact our office so we can maintain communication with your hospital team.   

 

What hospitals are affiliated with your practice?

Currently we are affiliated with Northwestern Memorial Medical Center and Swedish Covenant Hospital.
